Cancer – An Overview

Cancer is second only to cardiovascular disease as the leading cause of death in the Western world.

Although Cancer is primarily a disease of the elderly with more than 60% of deaths from cancer occurring in those over the age of 65, cancer can strike even the youngest of children.

Cancer appears to occur when the growth of cells in the body is out of control and cells divide too rapidly. Carcinomas can develop in almost any organ or tissue, such as the lung, colon, breast, skin, bones, or nerve tissue.

Most common sites are:

Prostrate 24%

Breast 13%

Lung 13%

Colon and Rectum 9%

Bladder 3%

Uterus 2.5%

The cause of Cancer is believed to be a combination of genetic factors and outside carcinogens such as tobacco, viruses, infection, asbestos, vinyl chloride, inappropriate diet.

Cancer often has no specific symptoms, so it is important that you limit your risk factors and undergo appropriate cancer screening. The signs and symptoms will depend on where the cancer is, the size of the tumor, and how much it affects the nearby organs or structures.

If a cancer spreads (metastasizes), then symptoms may appear in different parts of the body. As a tumour grows, it begins to push on nearby organs, blood vessels, and nerves. If the cancer is in a critical area, such as certain parts of the brain, even the smallest tumor can cause early symptoms.

But sometimes cancers start in places where it does not cause any symptoms until the cancer has grown quite large. Pancreatic cancers, for example, do not usually grow large enough to be felt from the outside of the body.

By the time a pancreatic cancer causes these signs or symptoms, it has usually reached an advanced stage.

A cancer may also cause symptoms common to many other problems, such as; fever, fatigue and weight loss. This may be because the cancer uses up much of the body’s energy or it may cause the release of substances which affect metabolism.

Some lung cancers make hormone-like substances that affect blood calcium levels, affecting nerves and muscles and causing weakness and dizziness.

It is important to know what some of the general (non-specific) signs and symptoms of cancer are, but remember that having any of these does not mean that you have cancer.

Most cancers can be treated and some cured, depending on the specific type, location, and stage. The earlier tumors are found, the better the prognosis.

A good example of the importance of finding cancer early is melanoma skin cancer. Skin cancer can be easy to remove if it has not grown deep into the skin, and the 5-year survival rate (percentage of people living at least 5 years after diagnosis) at this stage is nearly100%.

Screening for breast cancer with mammograms has been shown to reduce the average stage of diagnosis of breast cancer in a population.

Colorectal cancer can be detected through fecal occult blood testing and colonoscopy, which reduces both colon cancer incidence and mortality, presumably through the detection and removal of pre-malignant polyps.

Similarly, cervical cytology testing (using the Pap smear) leads to the identification and excision of precancerous lesions.

Testicular self-examination is recommended for men beginning at the age of 15 years to detect testicular cancer.

SIGNS and SYMPTOMS

Pain may be an early symptom with some cancers such as bone cancers or testicular cancer.

Long-term constipation, diarrhea, or a change in the size of the stool may be a sign of colon cancer.

Pain with urination, blood in the urine, or a change in bladder function (such as more frequent or less frequent urination) could be related to bladder or prostate cancer.

Skin cancers may bleed and look like sores that do not heal.

A long-lasting sore in the mouth could be an oral cancer and should be dealt with right away, especially in patients who smoke, chew tobacco, or frequently drink alcohol.

Sores on the penis or vagina may either be signs of infection or an early cancer, and should not be overlooked.

Unusual bleeding can happen in either early or advanced cancer.

Blood in the sputum (phlegm) may be a sign of lung cancer.

Blood in the stool (or a dark or black stool) could be a sign of colon or rectal cancer.

Blood in the urine may be a sign of bladder or kidney cancer.

A bloody discharge from the nipple may be a sign of breast cancer.

Many cancers can be felt through the skin, mostly in the breast, testicle, lymph nodes (glands), and the soft tissues of the body. A lump or thickening may be an early or late sign of cancer. Self examination is an important diagnostic measure, particularly for breast tumors.

While they commonly have other causes, indigestion or swallowing problems may be a sign of cancer of the esophagus, stomach, or pharynx (throat).

A cough that does not go away may be a sign of lung cancer.

A tumor may be suspected for a variety of reasons, but the definitive diagnosis of most malignancies must be confirmed by histological examination of the cancerous cells by a pathologist.

TREATMENT

Once diagnosed, cancer is usually treated with a combination of surgery, chemotherapy and radiotherapy.

Radiation therapy may be used to treat almost every type of solid tumor, including cancers of the brain, breast, cervix, larynx, lung, pancreas, prostate, skin, stomach, uterus, or soft tissue sarcomas.

Most forms of chemotherapy target all rapidly dividing cells and are not specific for cancer cells, although some degree of specificity may come from the inability of many cancer cells to repair DNA damage, while normal cells generally can.

Contemporary methods for generating an immune response against tumours include intravesical BCG immunotherapy for superficial bladder cancer, and use of interferons and other cytokines to induce an immune response in renal cell carcinoma and melanoma patients.

Pain medication, such as morphine and oxycodone, and anti-emetics, drugs to suppress nausea and vomiting, are very commonly used in patients with cancer-related symptoms. transmission and disease.

Advances in cancer research have made a vaccine designed to prevent cancer available. The vaccine protects against four HPV types, which together cause 70% of cervical cancers and 90% of genital warts.

Another animal study has shown the importance of having company to prevent cancer occurring and in reducing of the likelihood of death from cancer. It is clear that for both animals and humans having good friends definitely improves the quality of life, and now, possibly, lack of friends might even be a matter of life or death. However researchers are still unsure of many of the mechanisms by which social isolation might contribute to the development of cancer.

We all know of wonderful people who have had lots of friends who socialized a good deal and who still died of cancer. I am not saying that not having friends is the one cause of cancer.

But we need to acknowledge the increasing evidence that having good friends can be a buffer to both prevent cancer occurring in the first place and if it does occur, to improve ones chances of a better outcome – either a complete recovery or a longer time till death. My own research findings also come to the same conclusions.

I have never had a doctor ask me about my relationships with friends, or even within my family, and yet there is clear evidence of their importance to health. As I work with doctors I’ve asked a few of them why this lack of interest in patients’ sociability might be. Their response was that it is not their business to interfere, or even take an interest in, a patient’s relationships. It is outside their sphere of management.

As you are unlikely to hear from your doctor that good relationships are beneficial to your health right down to the cellular and organ level then you need to take the message on board from elsewhere and take responsibility for improving your relationships as part of your overall health care.

So if you know you are short on friends don’t take it badly. Friends die. They shift away. They or you can change. Or you might just look at your current friends and decide that they no longer contribute sufficient positive aspects to your life.

One of the patients in my big study had no friends at all when she first got cancer. Apart from a loving husband and two absent children she had no positive input from people into her life. She started to deal with her cancer by paying for human contact. She paid for a masseur, for time with a homeopath, joined a healing group and slowly made progress from there.

Another loner was a farmer who lived in the outback miles from anywhere. He gave up farming and moved into the city. His situation was different because he was naturally gregarious and made friends easily.

You don’t need lots of friends, somewhere between five and nine seems to work well from a health perspective. Family can count in the friend mix, but you are looking for people who contribute positively to your well-being and you to theirs. You are looking for at least one person who makes you feel significant, one who makes you feel connected and loved, one who can be trusted to help you and gives you a sense of security and someone who gives a sense of fun, enjoyment and variety in life.

As my study showed sometimes you must adapt, even to purchase the support you need and this is a positive alternative if your natural circle of friends is missing a category of need. It makes sense to have a good group of friends. You may not be thinking in terms of your own natural cancer therapies but friendships are good and improve your quality of life. Take the time to consider what your friends add to your life and if necessary set out to increase the number of positive people around you.

As organisms of impure substances must be expelled, women patients may experience a discharge; others may experience skin rashes, diarrhea or vomiting. This should not frighten anyone and may not continue for more than three days: indeed it is an indication that one is on the road to recovery.

Aloe Vera has very dramatic and impressive anti-cancer effects and has been demonstrated to enhance the immune system’s response to cancer, promote the growth of new and healthy cells, and reduce the overall viral load within the body thereby revitalizing the body in its fight against the cancer. Lung cancer may be the most tragic cancer because in most cases, it might have been prevented, 87% of lung cancer cases are caused by smoking. Lung cancer has long been the most common cause of cancer death in men and since 1987 it has also become the most common cause of cancer death in women. Lung cancer is the second most commonly occurring form of cancer in most western countries and although the lung cancer incidence is less common in developing countries, the rapid increase in the popularity of smoking will see the number of lung cancer sufferers in those countries quickly catch up with the western world.

Lung cancers can arise in any part of the lung, and 90%-95% of cancers of the lung are thought to arise from the epithelial, or lining cells of the larger and smaller airways (bronchi and bronchioles); for this reason, lung cancers are sometimes called bronchogenic carcinomas or bronchogenic cancers.
The most common type of lung cancers are epidermoid carcinoma, small cell carcinoma, adenocarcinoma and large cell carcinoma.

Most experts agree that lung cancer is attributable to inhalation of carcinogenic pollutants by a susceptible host. Who is most susceptible? Any smoker over the age of 40, especially if they began smoking before the age of 15, have smoked 20 or more for 20 years, or worked with or near asbestos. Two other factors also increase susceptibility: exposure to carcinogenic industrial and air pollutants (asbestos, uranium, arsenic, nickel, iron oxides, chromium, radio active dust, and coal dust.) and familial susceptibility.

SIGNS AND SYMPTOMS
Because early lung cancer usually produces no symptoms, the disease is often in an advanced stage when first diagnosed. Late stage signs are: with epidermoid and small cell carcinoma; smokers cough, hoarseness, wheezing, dyspnea, hemoptysis and chest pain. With adenocarcinoma and large cell carcinoma; fever, weakness, weight loss, anorexia and shoulder pain. In addition, hormone production which regulates various body functions may also be affected.

DIAGNOSIS
Firm diagnosis requires chest x rays, sputum cytology, CT scanning, bronchoscopy the examination of pleural fluid and biopsies. Other tests to detect metastasis include bone scans, bone marrow biopsy and CT scans of the brain and abdomen.

METASTASES
Lung cancer most often spreads to the liver, the adrenal glands, the bones, and the brain. Lung cancer that has metastasized to the bone causes bone pain, usually in the backbone (vertebrae), the thighbones, and the ribs. Lung cancer that spreads to the brain can cause difficulties with vision and weakness on one side of the body.
Lung cancer may grow into certain nerves in the neck, causing a droopy eyelid, small pupil, sunken eye, and reduced perspiration on one side of the face; together these symptoms are called Horner’s syndrome (see Autonomic Nervous System Disorders: Horner’s Syndrome). Lung cancer may grow directly into the esophagus, or it may grow near it and put pressure on it, leading to difficulty in swallowing. Lung cancer may also spread through the bloodstream to the liver, brain, adrenal glands, spinal cord, and bone.

TREATMENT
Treatment for lung cancer depends on the cancer’s specific cell type, how far it has spread, and the patient’s performance status. If investigations confirm lung cancer, CT scan and often positron emission tomography (PET) are used to determine whether the disease is localised and amenable to surgery or whether it has spread to the point where it cannot be cured surgically. Treatment is usually a combination of surgery, chemotherapy and radiation therapy.

Surgery is usually the first option. Chemotherapy can be used as a first line treatment for lung cancer or as additional treatment after surgery. Radiation therapy can be directed at your lung cancer from outside your body (external beam radiation) or it can be put inside needles, seeds or catheters and placed inside your body near the cancer (brachytherapy). Radiation therapy can be used alone or along with other lung cancer treatments. Radiation therapy can also be used to lessen side effects of lung cancer.

Treatment may not be as effective for patients with bone or liver metastases from lung cancer, excessive weight loss, ongoing cigarette use, or pre-existing medical conditions such as heart disease or emphysema. At some point, if you and your oncologist or primary care physician agree that treatment no longer is advisable, hospice care can provide comfort.

With more than 1 000 deaths a year, cancer of the cervix represents a major cause of cancer death in women in USA. In the beginning of its development cervic precancerous lesions are caused by virus of the papillomavirus (HPV) family.

Thanks to smear, it is possible to achieve a genuine screening. But according to a French team, the cancer risk assessment could be improved by supplementing it with a more precise identification of the virus.

Prevention

The cervicovaginal smear

The cervical smear is a simple consideration. It involves removing cells from the cervix by a simple painless scraping, and spread the levy on a slide. After fixing, the slides are examined under a microscope by a doctor specializing in this reading, the anatomical pathologist.

He describes in his cells report what he saw and can classify the smear in classes I to V, this is absolutely necessary, the description of cells must be sufficient to distinguish what is normal from what is suspect or frankly pathological.

Treatment

It uses radiation (brachytherapy utérovaginale and cobalt) and surgery. Chemotherapy is used in some cases.

Brachytherapy utérovaginale is to put a radioactive source in the uterine cavity and vagina in direct contact with the tumor. The principle of a homogeneous high dose in a small volume, is the basis of efficiency and good tolerance of brachytherapy. The applicator is set up under general anesthesia. Once in place, it is loaded into a second time through a projector sources. Dosimetry is done by computer.

At the early stage, the intervention may be limited among young women still wanting children to a simple conization or amputation of the cervix under carefully examination. Among the older woman, removal of the uterus (hysterectomy simple total) is performed.

In more advanced forms, a wider surgery is performed, and may be associated with external radiation from the small basin.

The signs of the disease

There are no symptoms. It is a discovery of cervical screening done in the framework of the annual gynecologic or during medical surveillance of an inflammatory condition or cervical dysplasia (polyp, ulceration etc…)

When the cancer start to grow, the patient usually consults for loss of blood (Metrorrhagia) often caused by vaginal discharge (white losses) These symptoms are usually painless.

Anyway cervicovaginal smears are performed, and a colposcopy, which will allow the biopsy. The speculum examination may also reveal that the cervix is injured or ulcerated.

Treatment of breast cancer is an extremely important issue – yet many women do not know their treatment options for breast cancer. What do most women know about treating breast cancer, and what can cancer hospitals do to help women make better informed choices?

Breast cancer is the second most common cancer worldwide, the leading cancer in women, and the second leading cause of death in women worldwide. It has an incidence rate more than twice that of cervical cancer and colorectal cancer, and three times as high as lung cancer in women. It follows that breast cancer is one of the most researched and publicized conditions – however, many groups in our society know little about their treatment options for breast cancer.
